- Relevant Imaging reports where available
- Correspondence from other treating practitioners involved in your care
- Details of your employment for Work related injuries

- From you at your Initial appointment with our registration form
- From an enquiry submission on our website
- Over the phone when booking an appointment
- From Medical Practitioner referrals or other Allied Health Correspondence
How do we use your Information:
We will only collect Personal Information that is relevant and necessary to safely provide our health services, and to communicate with others involved in your care.
We use this information to:
- Send Communications and reminders to you, such as appointment reminders and home exercise programs.
- For direct marketing email communications
- To share appropriate billing details with Health funds, Worksafe, TAC, DVA or Medicare when needed.
- To receive relevant imaging results related to your care
We do not on sell your personal information.
How we keep your information safe:
All team members are educated in Data Protection principles and Password Safety management.
Your information is stored in our Secure Practice Management system which is accessed by personal log in and 2-factor authentication. This data is protected by encryption and secure back ups, and all data is securely stored in data centres in Australia.
You can request to access copies of your Personal Information and Clinical records by signing a written Release consent form.
